Photography: Any person who attends an AzPA event grants permission to AzPA, its employees and agents (collectively “AzPA”) to record his or her visual/ audio images, including but not limited to, photographs, digital images, voices, sound or video recordings, audio clips, or accompanying written descriptions, and, without notifying such person, to use his or her name and such images for any purpose of AzPA, including advertisements for AzPA and its programs.
Refund and Cancellation Policy: Refund and/or cancellation requests must be made in writing to the Arizona Pharmacy Association office before May 14, 2018. A $50 service charge will apply to all cancellations. Substitutions are welcome. Refund checks will be mailed two weeks after the conclusion of the annual convention. No refund requests will be honored after May 14, 2018. AzPA reserves the right to cancel any activity because of in sufficient pre-registration as determined by the Executive Director or CE Administrator. In this case, all individuals pre-registered will be notified and issued a full refund.
